Field Behavioral Health Case Manager

- Develop and maintain relationships with patients and their families to provide compassionate and personalized care
- Conduct thorough assessments of patients' behavioral health needs and develop individualized care plans
- Collaborate with healthcare providers, social workers, and community resources to coordinate and advocate for patients' behavioral health needs
- Monitor and evaluate patients' progress and adjust care plans as needed
- Educate patients and families on available resources and treatment options
- Maintain accurate and timely documentation of patient care and progress
- Provide crisis intervention and support to patients in times of need
- Stay up-to-date on current trends and best practices in behavioral health care
- Act as a liaison between patients, families, and healthcare providers to ensure effective communication and continuity of care
- Adhere to all HIPAA regulations and maintain confidentiality at all times.
Bachelor's Degree In A Related Field Such As Social Work, Psychology, Or Counseling.
Minimum Of 2 Years Of Experience In Behavioral Health Case Management Or Related Field.
Knowledge And Understanding Of Mental Health And Substance Abuse Disorders, Treatment Modalities, And Community Resources.
Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With A Diverse Population.
Strong Organizational And Time Management Skills, With The Ability To Prioritize And Manage A Caseload Effectively.
