1. Conduct individualized nutrition assessments for patients, taking into account their medical history, dietary habits, and goals.
2. Develop personalized nutrition plans for patients based on their specific needs and goals.
3. Provide education and counseling to patients on healthy eating habits, including meal planning, portion control, and label reading.
4. Stay updated on the latest research and developments in the field of clinical nutrition to ensure the most current and evidence-based practices are used.
5. Collaborate with other healthcare professionals, such as physicians and dietitians, to develop comprehensive treatment plans for patients.
6. Monitor and track patients' progress towards their nutrition goals, making adjustments to their plans as needed.
7. Provide ongoing support and motivation to patients to help them stay on track with their nutrition plans.
8. Document all patient interactions and progress in a timely and accurate manner.
9. Participate in team meetings and case conferences to discuss patient progress and share knowledge and insights.
10. Maintain confidentiality and adhere to all HIPAA regulations in regards to patient information.
