  1. Conduct comprehensive medication therapy reviews for patients with cardiovascular diseases to ensure appropriate and effective drug therapy.
  2. Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with cardiovascular diseases.
  3. Serve as a drug information resource for healthcare providers and patients, providing education regarding medication therapy, potential drug interactions, and side effects.
  4. Monitor and assess the effectiveness of medication therapy for patients with cardiovascular diseases, making recommendations for adjustments as needed.
  5. Stay up-to-date on the latest developments and research in cardiology and medication therapy to ensure the most current and evidence-based care for patients.
  6. Develop and implement protocols and guidelines for the safe and effective use of medications in patients with cardiovascular diseases.
  7. Participate in quality improvement initiatives to identify areas for improvement and develop strategies to enhance patient outcomes.
  8. Provide education and training to healthcare providers and patients on medication use and management, including proper administration techniques and potential side effects.
  9. Serve as a liaison between the pharmacy department and other healthcare teams, promoting collaboration and interdisciplinary care.
  10. Continuously assess and evaluate medication therapy processes to identify opportunities for improvement and implement changes to enhance patient care.
  11. Maintain accurate and complete patient records, documenting all interventions and recommendations made.
  12. Adhere to all relevant laws, regulations, and policies regarding medication use, storage, and disposal.
  13. Participate in professional development activities to stay current in the field of cardiology and pharmacy practice.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.

  • Board Certification In Pharmacotherapy And/Or Cardiology Through The Board Of Pharmacy Specialties.

  • Completion Of A Pgy2 Residency In Cardiology Or Equivalent Clinical Experience.

  • Strong Knowledge Of Cardiology Pharmacotherapy, Including Anticoagulation, Lipid Management, And Heart Failure Medications.

  • Ability To Collaborate With Interdisciplinary Healthcare Teams And Provide Evidence-Based Recommendations For Patient Care.
