- Collaborate with healthcare team members to develop and implement individualized treatment plans for cardiology patients
- Conduct comprehensive medication reviews and provide recommendations for optimizing drug therapy
- Monitor patient response to medications and make appropriate adjustments as needed
- Educate patients on proper medication use and potential side effects
- Serve as a drug information resource for healthcare providers and patients
- Stay up-to-date on current research and developments in cardiovascular pharmacotherapy
- Participate in interdisciplinary rounds and meetings to discuss patient cases and contribute to treatment decisions
- Develop and implement protocols for medication use in the cardiology department
- Ensure compliance with all regulatory and accreditation standards related to medication management
- Document all pharmaceutical care activities accurately and in a timely manner
- Provide guidance and support to pharmacy technicians and other staff members as needed
- Maintain inventory of medications and supplies, and ensure proper storage and handling according to established guidelines
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Act as a liaison between the pharmacy department and other healthcare departments to promote seamless communication and collaboration.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: The Candidate Must Possess A Pharmd Degree To Be Eligible For The Position, As This Is A Specialized Role That Requires An In-Depth Understanding Of Cardiology Medications And Treatments.
Board Certification In Cardiology Pharmacy (Bccp): This Certification Demonstrates The Candidate's Advanced Knowledge And Skills In The Field Of Cardiology Pharmacy, Making Them A Valuable Asset To The Cardiology Team At Ascension Health.
Minimum Of 2 Years Of Clinical Pharmacy Experience: Ascension Health Is Seeking Candidates With Previous Experience In A Clinical Pharmacy Setting, Preferably In A Cardiology-Specific Role. This Experience Will Ensure That The Candidate Has A Strong Foundation In Clinical Knowledge And Skills.
