
Advanced Practice Provider NP PA - Palliative Care
We are seeking a compassionate and highly skilled Advanced Practice Provider (APP) to join our Palliative Care team at Ascension Health. As an APP specializing in Palliative Care, you will have the unique opportunity to provide comprehensive care and support to patients and families facing serious or life-limiting illnesses. - Provide compassionate and comprehensive care to patients and families facing serious or life-limiting illnesses.
- Collaborate effectively with a multidisciplinary team to develop and implement individualized care plans.
- Demonstrate a strong commitment to patient-centered care and advocate for the needs and preferences of patients and their families.
- Utilize advanced clinical skills and knowledge to assess, diagnose, and manage complex symptoms and provide relief from pain and discomfort.
- Conduct thorough patient assessments and review medical histories to determine appropriate treatment plans.
- Educate and support patients and families in understanding treatment options, managing symptoms, and making informed healthcare decisions.
- Maintain accurate and timely documentation of patient care in accordance with organizational policies and procedures.
- Stay current with advancements and best practices in Palliative Care through ongoing education and professional development.
- Adhere to all legal and ethical standards, including maintaining confidentiality of patient information.
- Collaborate with other healthcare providers and community resources to ensure continuity of care for patients.
- Act as a resource and mentor to other healthcare professionals and students in training.
- Participate in quality improvement initiatives to enhance the delivery of Palliative Care services.
- Comply with all regulatory and accreditation requirements related to APP practice and Palliative Care services.
- Contribute to a positive and collaborative work environment by actively participating in team meetings and sharing knowledge and expertise with colleagues.
- Serve as an ambassador for Ascension Health and promote the organization’s mission and values in all interactions with patients, families, and colleagues.
Advanced Degree: A Minimum Of A Master's Degree In Nursing Or Physician Assistant Studies Is Required For This Position. This Ensures That The Candidate Has Received Specialized Education And Training In Advanced Practice Nursing Or Physician Assistant Practice.
State Licensure And Certification: The Candidate Must Hold A Current, Unrestricted License To Practice As A Nurse Practitioner Or Physician Assistant In The State Where They Will Be Working. Additionally, Certification As A Nurse Practitioner (Np) Or Physician Assistant (Pa) In Palliative Care Is Preferred.
Clinical Experience: A Minimum Of 2-3 Years Of Clinical Experience In Palliative Care Is Required For This Role. This Ensures That The Candidate Has A Strong Foundation In Providing Comprehensive And Compassionate Care To Patients With Serious Or Life-Threatening Illnesses.
Communication And Interpersonal Skills: As An Advanced Practice Provider In Palliative Care, Strong Communication And Interpersonal Skills Are Essential. The Candidate Must Be Able To Effectively Communicate With Patients, Families, And Interdisciplinary Teams To Provide Holistic And Patient-Centered Care.
