  1. Conduct comprehensive patient assessments and develop individualized care plans for patients with heart failure.
  2. Collaborate with physicians, nurses, and other healthcare professionals to provide high-quality and evidence-based care.
  3. Monitor and manage patient progress, including medication management and follow-up care.
  4. Educate patients and their families on managing heart failure and promoting healthy lifestyle choices.
  5. Participate in team meetings and contribute to the development and implementation of new protocols and procedures.

Job Qualifications
  • Master's Degree In Nursing Or Physician Assistant Studies With A Focus On Advanced Practice In Cardiology Or Heart Failure Management.

  • Current Certification As A Nurse Practitioner Or Physician Assistant With Specialty Certification In Cardiology Or Heart Failure.

  • Minimum Of 2-3 Years Of Experience As An Advanced Practice Provider In An Inpatient Setting, With A Focus On Heart Failure Management.

  • Proficiency In Advanced Cardiac Assessment, Diagnostic Testing Interpretation, And Medication Management For Heart Failure Patients.

  • Strong Communication And Collaborative Skills, With The Ability To Work Closely With A Multidisciplinary Team To Develop And Implement Comprehensive Treatment Plans For Heart Failure Patients.
