1. Provide high-quality clinical care to patients in their homes.
2. Conduct initial and ongoing assessments of patients' health status and develop individualized care plans.
3. Administer medications and treatments as prescribed by physicians.
4. Monitor and record patients' progress and document any changes in their condition.
5. Collaborate with interdisciplinary healthcare team members to ensure comprehensive and coordinated care.
6. Educate patients and their families about their healthcare needs and provide them with necessary resources and tools.
7. Maintain accurate and timely documentation of all patient care activities.
