RN Hospice - Case Manager

We are currently seeking a talented and dedicated Registered Nurse (RN) to join our team as a Hospice Case Manager. - Provide compassionate and personalized care to patients in the comfort of their own homes.
- Serve as a patient advocate, ensuring their needs and wishes are heard and respected.
- Coordinate and manage care plans for patients at the end-of-life stage.
- Collaborate with interdisciplinary team members, including physicians, social workers, and spiritual care providers, to develop and implement comprehensive care plans.
- Conduct thorough patient assessments to identify physical, emotional, and spiritual needs.
- Administer medications, treatments, and procedures as ordered by physicians.
- Monitor and evaluate patient progress, adjusting care plans as needed to ensure optimal outcomes.
- Educate patients and their families on end-of-life care, symptom management, and pain control.
- Provide emotional support and counseling to patients and their families.
- Maintain accurate and timely documentation of patient care and progress.
- Ensure all care provided meets company, state, and federal regulations and standards.
- Participate in performance improvement activities to enhance patient care and outcomes.
- Collaborate with community resources to provide additional support and resources for patients and their families.
- Attend and actively participate in team meetings, patient conferences, and in-service education programs.
- Stay updated on current hospice care practices and trends, incorporating them into daily practice.
Active Rn License: The First And Most Important Qualification For A Rn Hospice - Case Manager At Amedisys Is Having An Active Registered Nurse (Rn) License. This Ensures That The Candidate Has Met All The Necessary Educational And Training Requirements To Practice As A Nurse.
Hospice Experience: Amedisys Is A Leading Hospice Care Provider, And They Require Their Rn Hospice - Case Managers To Have Previous Experience In Hospice Care. This Includes Knowledge Of Pain And Symptom Management, End-Of-Life Care, And Communication With Patients And Their Families.
Case Management Skills: The Rn Hospice - Case Manager Is Responsible For Coordinating And Managing The Care Of Hospice Patients. This Requires Strong Case Management Skills, Including Assessment, Care Planning, And Collaboration With Other Healthcare Professionals.
