We are currently seeking a talented and dedicated Registered Nurse (RN) to join our team as a Hospice Case Manager. In this role, you will have the opportunity to make a meaningful impact on the lives of patients and their families during their end-of-life journey. As a member of our interdisciplinary team, you will provide comprehensive nursing care, serve as a patient advocate, and coordinate care plans to ensure the best possible outcomes.   1. Provide compassionate and personalized care to patients in the comfort of their own homes.
  2. Serve as a patient advocate, ensuring their needs and wishes are heard and respected.
  3. Coordinate and manage care plans for patients at the end-of-life stage.
  4. Collaborate with interdisciplinary team members, including physicians, social workers, and spiritual care providers, to develop and implement comprehensive care plans.
  5. Conduct thorough patient assessments to identify physical, emotional, and spiritual needs.
  6. Administer medications, treatments, and procedures as ordered by physicians.
  7. Monitor and evaluate patient progress, adjusting care plans as needed to ensure optimal outcomes.
  8. Educate patients and their families on end-of-life care, symptom management, and pain control.
  9. Provide emotional support and counseling to patients and their families.
  10. Maintain accurate and timely documentation of patient care and progress.
  11. Ensure all care provided meets company, state, and federal regulations and standards.
  12. Participate in performance improvement activities to enhance patient care and outcomes.
  13. Collaborate with community resources to provide additional support and resources for patients and their families.
  14. Attend and actively participate in team meetings, patient conferences, and in-service education programs.
  15. Stay updated on current hospice care practices and trends, incorporating them into daily practice.

Job Qualifications
  • Active Rn License: The First And Most Important Qualification For A Rn Hospice - Case Manager At Amedisys Is Having An Active Registered Nurse (Rn) License. This Ensures That The Candidate Has Met All The Necessary Educational And Training Requirements To Practice As A Nurse.

  • Hospice Experience: Amedisys Is A Leading Hospice Care Provider, And They Require Their Rn Hospice - Case Managers To Have Previous Experience In Hospice Care. This Includes Knowledge Of Pain And Symptom Management, End-Of-Life Care, And Communication With Patients And Their Families.

  • Case Management Skills: The Rn Hospice - Case Manager Is Responsible For Coordinating And Managing The Care Of Hospice Patients. This Requires Strong Case Management Skills, Including Assessment, Care Planning, And Collaboration With Other Healthcare Professionals.

  • Communication And Interpersonal Skills: As A Hospice Case Manager, Effective Communication And Interpersonal Skills Are Essential. The Ability To Communicate Compassionately And Effectively With Patients, Families, And Other Healthcare Professionals Is Crucial In This Role.

  • Cultural Competency: Amedisys Serves A Diverse Patient Population, And They Value Cultural Competency In Their Employees. The Rn Hospice - Case Manager Should Be Able To Provide Culturally Sensitive Care And Understand The Unique Needs And Beliefs Of Different Cultures.
