1. Provide high-quality and compassionate care to home health patients in accordance with Amedisys standards and policies.
2. Develop and implement individualized care plans for patients based on their specific needs and goals.
3. Conduct initial assessments and ongoing evaluations of patients' conditions to determine appropriate care and treatment.
4. Administer medications and treatments as prescribed by physicians.
5. Educate patients and their families on disease management, self-care techniques, and available community resources.
6. Communicate effectively with patients, families, and other healthcare team members to ensure coordinated and comprehensive care.
7. Maintain accurate and thorough documentation of patient care and progress.
8. Collaborate with other healthcare professionals to ensure the best possible outcomes for patients.
9. Maintain a safe and clean work environment, adhering to infection control and safety protocols.
10. Stay current with the latest developments and advancements in home health care and incorporate them into practice.
11. Participate in the orientation and training of new team members.
12. Adhere to all federal, state, and local regulations and laws governing nursing practice and home health care.
13. Demonstrate professionalism and a strong commitment to ethical standards in all interactions with patients, families, and colleagues.
14. Continuously strive to improve patient outcomes and satisfaction.
15. Perform other duties as assigned by the supervisor.
