1. Oversee and manage the care of patients receiving home health services.
2. Lead and develop a team of nurses, providing guidance and support.
3. Ensure that all patient care is provided in accordance with company policies and procedures, as well as state and federal regulations.
4. Conduct regular assessments of patient needs, and collaborate with the interdisciplinary team to develop and implement individualized care plans.
5. Monitor the quality of care provided by the team, identifying areas for improvement and implementing corrective actions as needed.
6. Maintain accurate and timely documentation of patient care, including assessments, progress notes, and medication administration records.
7. Communicate effectively with patients, families, and caregivers to provide education and support and ensure their understanding of the care plan.
8. Collaborate with physicians and other healthcare providers to coordinate and manage patient care.
9. Monitor and ensure compliance with infection control, safety, and emergency protocols.
10. Participate in performance improvement initiatives and provide feedback to promote continuous improvement of patient care.
11. Serve as a role model and mentor for nursing staff, fostering a positive and professional work environment.
12. Stay current on best practices and trends in home health care, and share knowledge and expertise with the team.
13. Adhere to all professional, ethical, and legal standards of nursing practice.
14. Represent the company positively in the community and maintain positive relationships with healthcare partners.
15. Participate in on-call rotation and respond to after-hours patient care needs as required.
