- Communicate with patients and families: The RN Case Manager will be responsible for communicating with patients and their families, providing updates on treatment plans and addressing any concerns or questions they may have.
- Collaborate with healthcare team: The RN Case Manager will work closely with other healthcare professionals, including physicians, nurses, and therapists, to develop and implement comprehensive care plans for patients.

Valid Rn License: A Rn Case Manager At Amedisys Must Possess A Valid Registered Nurse (Rn) License In The State Where They Will Be Practicing.
Bachelor's Degree In Nursing: Amedisys Prefers Candidates With A Bachelor's Degree In Nursing (Bsn) As It Provides A More Comprehensive Education And Prepares Nurses For Leadership Roles.
Minimum Of 2 Years Of Clinical Experience: Candidates Should Have At Least 2 Years Of Experience Working As A Nurse In A Clinical Setting, Preferably In A Home Health Or Hospice Care Environment.
Case Management Certification: Amedisys Prefers Candidates With A Case Management Certification (Ccm) Or Other Relevant Certifications In Case Management, Such As From The American Nurses Credentialing Center (Ancc).
Strong Communication And Organizational Skills: A Rn Case Manager Must Possess Excellent Communication And Organizational Skills To Effectively Coordinate Care For Patients And Collaborate With Other Members Of The Healthcare Team.
