RN Case Manager

• Develop and implement individualized care plans for patients, in collaboration with the interdisciplinary team, that address their physical, emotional, and psychosocial needs
• Conduct initial and ongoing assessments of patients' conditions, and update care plans accordingly
• Coordinate with physicians, therapists, and other healthcare professionals to ensure effective and timely delivery of care
• Monitor and evaluate patient progress, and make necessary adjustments to care plans as needed
• Provide direct nursing care to patients as needed, including administering medications, performing treatments, and providing education and support to patients and their families
• Serve as a liaison between patients, their families, and healthcare providers, ensuring effective communication and collaboration
• Advocate for patients and their needs, and ensure their rights and preferences are respected and upheld
• Maintain accurate and up-to-date documentation of patient care and progress, in compliance with company policies and regulatory requirements
• Participate in team meetings and case conferences to discuss patient care and develop effective treatment plans
• Keep abreast of current healthcare practices and regulations, and participate in ongoing training and professional development opportunities.
