Job Description

We are seeking a dedicated and experienced registered nurse who is passionate about hospice care and can provide exceptional nursing services to our patients.   1. Provide compassionate and quality care to patients and their families facing end-of-life care.
  2. Serve as the primary point of contact for patients and families, ensuring their needs are met and concerns are addressed in a timely and respectful manner.
  3. Develop and implement individualized care plans in collaboration with the interdisciplinary team and in accordance with hospice regulations and standards.
  4. Conduct comprehensive assessments of patient needs, including physical, emotional, and psychosocial factors, and regularly re-evaluate and adjust care plans accordingly.
  5. Administer medications and treatments as prescribed by the physician and monitor patient's response and progress.
  6. Educate patients and families on disease processes, medications, and self-care techniques to promote independence and improve quality of life.
  7. Coordinate and communicate with other healthcare professionals involved in the patient's care, including physicians, therapists, and social workers.
  8. Advocate for patients' rights and choices, ensuring their wishes are respected and honored.
  9. Document all patient care activities accurately and in a timely manner, adhering to HIPAA and other regulatory requirements.
  10. Participate in interdisciplinary team meetings and case conferences to discuss patient care and address any concerns or challenges.
  11. Provide support and guidance to nursing staff, including LPNs and CNAs, and assist with their training and development.
  12. Maintain current knowledge of hospice care standards and best practices, and participate in ongoing education and training to enhance clinical skills.
  13. Adhere to Amedisys policies and procedures and maintain confidentiality in all aspects of job performance.
  14. Foster a positive and collaborative work environment, promoting teamwork and open communication among team members.
  15. Uphold the values and mission of Amedisys, serving as an ambassador for the organization in interactions with patients, families, and the community.

Job Qualifications
  • Active Rn License: The First And Most Essential Qualification For An Rn Case Manager Hospice Job At Amedisys Is An Active Rn License. This Ensures That The Candidate Has The Necessary Knowledge And Skills To Provide Quality Care To Hospice Patients.

  • Minimum Of 2 Years Of Clinical Experience: Amedisys Requires Candidates For This Role To Have A Minimum Of 2 Years Of Clinical Experience As A Registered Nurse. This Ensures That The Candidate Has A Solid Foundation In Nursing And Is Able To Handle The Responsibilities Of A Case Manager In A Hospice Setting.

  • Hospice Or Palliative Care Certification: Amedisys Prefers Candidates Who Have A Certification In Hospice Or Palliative Care. This Demonstrates That The Candidate Has A Specialized Understanding Of The Unique Needs Of Hospice Patients And Their Families.

  • Excellent Communication And Interpersonal Skills: As A Case Manager, The Rn Will Be Responsible For Coordinating Care With The Patient, Their Family, And Other Healthcare Professionals. Therefore, Strong Communication And Interpersonal Skills Are A Must For This Position.

  • Compassionate And Empathetic Attitude: Working In Hospice Care Requires A High Level Of Compassion And Empathy. Amedisys Looks For Candidates Who Have A Genuine Desire To Provide Comfort And Support To Patients And Their Families During Their End-Of-Life Journey.
