- Provide compassionate and personalized care to patients in the end-of-life journey.
- Utilize strong background in nursing to assess, plan, implement and evaluate patient care.
- Work closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans.
- Coordinate and manage all aspects of patient care, including physical, emotional, and spiritual needs.
- Conduct regular assessments and reassessments of patient's condition and progress.
- Collaborate with physicians and other healthcare professionals to ensure continuity of care.
- Educate patients and families on disease process, treatment options, and end-of-life care.
- Administer medications and perform treatments as prescribed by the physician.
- Monitor patient's response to treatment and report any changes or concerns to the physician.
- Maintain accurate and timely documentation of patient care and progress.
- Advocate for patients' rights and ensure their wishes are respected.
- Provide emotional support and counseling to patients and families.
- Participate in interdisciplinary team meetings to discuss patient care and make recommendations.
- Adhere to all company policies, procedures, and regulations.
- Continuously strive to improve the quality of care provided to patients.
- Keep abreast of current hospice and palliative care practices and trends.
- Act as a liaison between patients, families, and healthcare providers.
- Follow infection control and safety protocols to ensure a safe and healthy environment for patients.
- Maintain confidentiality of all patient information.
- Maintain a positive and professional attitude while providing care to patients and their families.
Active Rn License: A Rn Case Manager Hospice At Amedisys Must Possess An Active And Unrestricted Rn License In The State Where They Will Be Practicing.
Minimum Of 2 Years Of Nursing Experience: Amedisys Seeks Candidates With At Least 2 Years Of Nursing Experience, Preferably In Hospice Or Home Health Care. This Demonstrates The Necessary Clinical Knowledge And Skills For The Role.
Knowledge Of Hospice Care Regulations And Guidelines: A Rn Case Manager Hospice Must Have A Strong Understanding Of Federal And State Regulations For Hospice Care, As Well As The Guidelines Set By Medicare And Medicaid.
Excellent Communication And Interpersonal Skills: This Role Requires Frequent Communication With Patients, Families, And Interdisciplinary Team Members. Amedisys Looks For Candidates With Strong Communication And Interpersonal Skills To Effectively Navigate These Interactions.
Case Management Experience: Amedisys Prefers Candidates With Prior Experience In Case Management, Specifically In Hospice Or Home Health. This Includes Experience Coordinating Care, Developing Care Plans, And Collaborating With Other Healthcare Professionals To Ensure Quality Patient Outcomes.
