- Provide comprehensive and compassionate hospice care to patients in their end-of-life journey, while also supporting their families.
- Develop and implement individualized care plans based on patients' needs and goals.
- Conduct regular assessments of patients' physical, emotional, and spiritual well-being.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patients' progress and any changes in their condition.

Active Rn License In The State Of Employment: A Qualified Candidate For An Rn Case Manager Hospice Job At Amedisys Must Hold A Valid And Active Registered Nurse License In The State Where The Hospice Agency Is Located.
Minimum Of 2 Years Of Nursing Experience: Amedisys Requires Candidates To Have At Least 2 Years Of Experience Working As A Registered Nurse, Preferably In An Acute Care Or Hospice Setting.
Knowledge Of Hospice Care Regulations And Processes: The Ideal Candidate Should Have A Thorough Understanding Of Hospice Care Regulations, Policies, And Procedures, As Well As Experience With Medicare And Medicaid Guidelines.
Excellent Communication And Interpersonal Skills: As A Case Manager, The Rn Will Be Responsible For Coordinating And Communicating With Patients, Families, And Interdisciplinary Team Members. Therefore, Strong Communication And Interpersonal Skills Are Essential For Success In This Role.
