  1. Develop and implement individualized care plans for patients, in collaboration with the interdisciplinary team, to ensure high-quality and compassionate care.
  2. Conduct initial and ongoing assessments of patients' physical, emotional, and psychosocial needs to determine appropriate care plan interventions.
  3. Monitor and evaluate the progress of patients' care plans, making necessary adjustments as needed to ensure optimal outcomes.
  4. Communicate effectively with patients, families, and healthcare team members to coordinate and provide continuity of care.
  5. Provide education and support to patients and their families regarding their care plans, including disease management, medication adherence, and self-care techniques.
  6. Document all patient care activities accurately and timely in accordance with company policies and procedures.
  7. Collaborate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  8. Adhere to all regulatory and legal requirements, maintaining patient confidentiality and privacy at all times.
  9. Participate in ongoing training and professional development activities to maintain current knowledge and skills in the field of nursing and case management.
  10. Serve as an advocate for patients and their families, ensuring their needs and preferences are addressed in their care plans.
  11. Demonstrate a commitment to providing exceptional customer service and fostering a positive and supportive work environment.
  12. Participate in quality improvement initiatives and contribute to the overall success and growth of the organization.
  13. Maintain a high level of professionalism, integrity, and ethical standards in all job-related activities.

Job Qualifications
  • Current Rn License: Amedisys Requires All Rn Case Managers To Hold A Valid And Active Registered Nurse License In The State Where They Will Be Practicing. This License Must Be Maintained Throughout Employment.

  • Bachelor's Degree In Nursing: Amedisys Prefers Candidates For Rn Case Manager Positions To Have A Bachelor Of Science In Nursing (Bsn) Degree. This Higher Level Of Education Provides A Strong Foundation For The Complex And Varied Responsibilities Of A Case Manager.

  • Minimum Of 2 Years Of Clinical Experience: Amedisys Seeks Candidates Who Have At Least 2 Years Of Experience Working As A Registered Nurse In A Clinical Setting. This Experience Helps Ensure That The Individual Has A Strong Clinical Background And Is Well-Equipped To Handle The Demands Of The Case Manager Role.

  • Strong Communication And Interpersonal Skills: As A Case Manager, The Rn Will Be Working Closely With Patients, Their Families, And Other Healthcare Professionals. Amedisys Values Individuals Who Possess Excellent Communication And Interpersonal Skills To Effectively Collaborate With The Entire Care Team.

  • Knowledge Of Medicare And Medicaid Regulations: Amedisys Provides Care Primarily To Medicare And Medicaid Patients, So Knowledge Of The Regulations And Requirements For These Programs Is Essential For Rn Case Managers. This Includes Understanding Of Reimbursement Policies, Documentation Guidelines, And Criteria For Certifying Patients For Home Care Services.
