  1. Provide compassionate and holistic care to patients and their families during their end-of-life journey.
  2. Serve as a supportive resource for patients and their loved ones, addressing their emotional, spiritual, and physical needs.
  3. Complete comprehensive assessments of patients' medical, psychosocial, and spiritual needs.
  4. Develop and implement personalized care plans for each patient, including symptom management and end-of-life planning.
  5. Coordinate with interdisciplinary team members to ensure all aspects of patient care are addressed.
  6. Collaborate with physicians and other healthcare professionals to provide the best possible care for patients.
  7. Educate patients and their families about hospice care and the end-of-life process.
  8. Monitor and document patients' progress and communicate any changes to the interdisciplinary team.
  9. Provide emotional support and guidance to patients and their families, as well as assistance with coping and grief.
  10. Adhere to all hospice care regulations and standards, as well as company policies and procedures.
  11. Participate in ongoing education and training to maintain current knowledge and skills in hospice care.
  12. Maintain accurate and timely patient records and documentation.
  13. Serve as an advocate for patients and their families, ensuring their wishes are respected and their needs are met.
  14. Collaborate with community resources and organizations to provide additional support and services for patients and their families.
  15. Maintain a professional and compassionate demeanor at all times, demonstrating empathy and respect for patients and their families.

Job Qualifications
  • Current Rn License: The First And Most Important Qualification For A Registered Nurse Hospice- Case Manager At Amedisys Is A Valid And Current Registered Nursing License. This Ensures That The Individual Has The Necessary Education And Training To Provide Safe And Effective Care To Hospice Patients.

  • Hospice Experience: Amedisys Requires A Minimum Of 2 Years Of Experience Working In A Hospice Setting For Their Registered Nurse Hospice-Case Manager Position. This Demonstrates The Candidate's Ability To Provide Compassionate End-Of-Life Care And Familiarity With The Unique Challenges Of Working In Hospice.

  • Case Management Skills: As A Case Manager, The Rn Hospice-Case Manager At Amedisys Is Responsible For Coordinating And Overseeing The Care Of Patients. Therefore, Strong Case Management Skills, Including The Ability To Assess, Plan, Implement, And Evaluate Care, Are Essential For This Role.

  • Communication And Interpersonal Skills: The Registered Nurse Hospice-Case Manager At Amedisys Must Have Excellent Communication And Interpersonal Skills To Effectively Collaborate With The Interdisciplinary Team, Patients, And Their Families. This Includes The Ability To Provide Emotional Support And Education To Patients And Their Families During A Difficult Time.

  • Knowledge Of Hospice Regulations: Amedisys Requires Their Registered Nurse Hospice-Case Manager To Have A Thorough Understanding Of Hospice Regulations And Guidelines. This Includes Knowledge Of Medicare And Medicaid Regulations, As Well As State And Federal Laws Related To Hospice Care.
