- Provide personalized and holistic care to patients in their homes during their end-of-life journey.
- Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Administer medications, treatments, and procedures according to physicians’ orders and hospice policies.
- Monitor patients’ progress and make necessary adjustments to their care plans.
- Educate patients and their families on disease processes, treatment options, and end-of-life care.
- Serve as an advocate and liaison between patients, families, and healthcare providers.
- Coordinate and communicate with other healthcare professionals involved in the patient’s care.
- Provide emotional support and counseling to patients and their families.
- Document all patient care activities accurately and timely in electronic medical records.
- Ensure compliance with all state and federal regulations, as well as hospice policies and procedures.
- Participate in interdisciplinary team meetings to discuss patient care and contribute to care planning.
- Attend continuing education programs to maintain and enhance clinical skills and knowledge.
- Maintain confidentiality and uphold ethical standards in all aspects of job performance.
- Collaborate with community resources to provide additional support and resources to patients and their families.
Bachelor's Degree In Nursing (Bsn): Amedisys Requires All Registered Nurse Hospice - Case Managers To Hold A Bsn Degree From An Accredited Institution. This Educational Background Provides Nurses With A Strong Foundation In Nursing Principles And Prepares Them For The Complexities Of Managing Hospice Care.
Current Rn License: All Candidates For The Registered Nurse Hospice - Case Manager Position Must Hold A Valid Rn License In The State Where They Will Be Practicing. This Ensures That The Nurse Is Legally Able To Provide Nursing Care And Is Up-To-Date On State-Specific Regulations And Practices.
Hospice And Palliative Care Certification: Amedisys Prefers Candidates Who Have Obtained Certification In Hospice And Palliative Care. This Certification Demonstrates A Nurse's Specialized Knowledge And Skills In Providing Care To Patients With Life-Limiting Illnesses.
At Least 2 Years Of Nursing Experience: Amedisys Looks For Candidates With At Least 2 Years Of Experience In A Clinical Setting, Preferably In Hospice Or Palliative Care. This Experience Enables The Nurse To Effectively Manage Complex Cases And Provide Quality Care To Patients And Their Families.
Strong Communication And Interpersonal Skills: As A Hospice - Case Manager, The Nurse Will Be Working Closely With Patients, Their Families, And A Multidisciplinary Team. Therefore, Strong Communication And Interpersonal Skills Are Essential To Effectively Coordinate And Deliver Care, As Well As Provide Emotional Support To Patients And Their Loved Ones During A Difficult Time.
