
Registered Nurse Hospice - Case Manager

- Provide high-quality and compassionate care to hospice patients in their homes.
- Serve as the primary point of contact for patients and their families, effectively communicating and addressing any concerns or needs.
- Coordinate and collaborate with the interdisciplinary hospice team to develop and implement a care plan that meets the physical, emotional, and spiritual needs of patients.
- Conduct thorough assessments of patients' conditions and provide appropriate interventions and treatments.
- Monitor and document patients' progress and make necessary adjustments to the care plan as needed.
- Educate patients and their families on the hospice care process, including medication management, symptom management, and end-of-life support.
- Ensure compliance with all state and federal regulations, as well as Amedisys policies and procedures.
- Maintain accurate and timely documentation of patient care in electronic medical records.
- Participate in interdisciplinary team meetings and provide input and recommendations for patient care.
- Continuously strive for professional development and stay updated on best practices in hospice care.
- Uphold Amedisys' mission and values, and represent the company in a professional and compassionate manner.
- Act as an advocate for patients and their families, ensuring their needs and wishes are respected and met.
- Collaborate with community resources and agencies to provide additional support and services for patients.
- Maintain confidentiality of all patient information and adhere to HIPAA guidelines.
- Participate in on-call rotation as needed.
- Undertake any other duties and responsibilities as assigned by the hospice manager.
Current Rn License: The First And Most Important Qualification For A Registered Nurse Hospice-Case Manager At Amedisys Is A Valid And Current Rn License. This License Demonstrates That The Individual Has Completed The Necessary Education And Training To Provide High-Quality Nursing Care.
Hospice Experience: Amedisys Is Specifically Looking For Candidates With Previous Experience Working In Hospice Care. This Experience Can Include Direct Patient Care, Case Management, Or Other Related Roles Within A Hospice Setting. This Experience Shows That The Candidate Has A Strong Understanding Of The Unique Needs And Challenges Of Hospice Care.
Case Management Skills: As A Case Manager, The Rn Will Be Responsible For Coordinating And Overseeing The Care Of Hospice Patients. Therefore, Amedisys Is Seeking Candidates With Strong Case Management Skills, Including The Ability To Assess Patient Needs, Develop Care Plans, And Communicate Effectively With Interdisciplinary Teams.
