Registered Nurse Case Manager RN Hospice

- Provide compassionate and exceptional nursing care to patients in hospice care.
- Utilize strong organizational skills to effectively manage and coordinate patient care.
- Communicate effectively with patients, families, and healthcare team members to ensure smooth and coordinated care.
- Serve as the primary point of contact for patients and families, addressing any concerns or needs that may arise.
- Participate in interdisciplinary team meetings to develop and review patient care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor and assess patient's physical and emotional well-being, and provide appropriate interventions as needed.
- Educate patients and families on disease process, treatment options, and end-of-life care.
- Collaborate with other healthcare professionals to ensure comprehensive and holistic care for patients.
- Maintain accurate and timely documentation of patient care and progress.
- Adhere to all regulatory and compliance standards.
- Continuously strive to improve patient outcomes and satisfaction.
- Stay updated on current trends, research, and advancements in hospice care.
- Demonstrate a commitment to the mission and values of Amedisys.
Current Rn License: Amedisys Requires All Applicants For The Rn Case Manager Hospice Position To Have A Valid And Unrestricted Registered Nursing License In The State Where They Will Be Practicing.
Minimum Of 2 Years Of Nursing Experience: To Be Considered For This Role, Candidates Must Have At Least 2 Years Of Experience Working As A Registered Nurse, Preferably In A Hospice Or Home Health Setting.
Knowledge Of Hospice Care And Regulations: The Ideal Candidate For This Role Should Have A Strong Understanding Of Hospice Care And Regulations, Including Medicare Guidelines And State-Specific Requirements.
Excellent Communication And Interpersonal Skills: As A Case Manager, The Rn Will Be Responsible For Coordinating Care And Communicating With Patients, Families, And Other Healthcare Professionals. Strong Communication And Interpersonal Skills Are Essential For Success In This Role.
Ability To Work Independently And As Part Of A Team: Amedisys Values Self-Motivated Individuals Who Can Work Independently And Make Sound Clinical Decisions. However, The Rn Case Manager Hospice Will Also Collaborate With A Multidisciplinary Team, So The Ability To Work Well With Others Is Crucial.
