
Registered Nurse Case Manager-Hospice

- Provide compassionate and comprehensive care to patients and their families in the hospice setting.
- Serve as a point of contact and advocate for patients and their families throughout the end-of-life care process.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs and develop individualized care plans.
- Coordinate and collaborate with interdisciplinary team members to ensure holistic and coordinated care.
- Administer medications, treatments, and procedures according to physician orders and established protocols.
- Monitor patients' conditions and report any changes or concerns to the physician.
- Educate patients and families on end-of-life care and provide emotional support.
- Maintain accurate and up-to-date patient records and documentation.
- Comply with all legal and ethical standards and maintain patient confidentiality.
- Participate in ongoing education and professional development activities to enhance knowledge and skills.
- Collaborate with community resources and healthcare providers to ensure continuity of care.
- Adhere to all company policies and procedures and promote a positive and professional work environment.

- Proactively identify and address potential barriers to effective hospice care.
- Provide guidance and support to nursing assistants and other team members as needed.

- Act as a liaison between patients, families, and the hospice team.
- Attend and actively participate in team meetings and case conferences.
- Continuously evaluate and improve the quality of care provided to patients and families.
Bachelor's Degree In Nursing: Amedisys Prefers Candidates With A Bachelor's Degree In Nursing, As It Demonstrates A Higher Level Of Education And Knowledge In The Field.
Valid Rn License: Amedisys Requires All Registered Nurse Case Managers-Hospice To Have A Current And Active Rn License In The State Where They Will Be Working.
Hospice Experience: A Minimum Of 2 Years Of Experience In A Hospice Setting Is Preferred For This Role. This Ensures That The Candidate Has A Strong Understanding Of The Unique Needs And Challenges Of Hospice Patients.
Case Management Experience: Amedisys Is Looking For Candidates With Previous Experience In Case Management, As It Is A Key Aspect Of The Job. This Includes Coordinating Care, Developing Care Plans, And Communicating With Other Healthcare Professionals.
Interpersonal And Communication Skills: As A Registered Nurse Case Manager-Hospice, The Ability To Communicate Effectively With Patients, Families, And Other Healthcare Professionals Is Crucial.
