  1. Provide compassionate and high-quality patient care, utilizing clinical skills and expertise, to improve the health and well-being of our patients.
  2. Serve as a dedicated case manager, responsible for coordinating and managing the care of patients under our care.
  3. Conduct thorough assessments of patient needs and develop individualized care plans to meet those needs.
  4. Collaborate with interdisciplinary healthcare team members to ensure effective and coordinated patient care.
  5. Monitor and evaluate patient progress, making adjustments to care plans as necessary.
  6. Communicate regularly with patients and their families to provide education, support, and guidance.
  7. Maintain accurate and up-to-date patient records and documentation.
  8. Adhere to all company policies, procedures, and standards of care.
  9. Stay current on industry best practices and participate in ongoing education and training opportunities.
  10. Serve as an advocate for patients, ensuring they receive the highest level of care and support.
  11. Maintain a high level of professionalism and ethical standards in all interactions with patients, colleagues, and members of the healthcare community.
  12. Demonstrate strong organizational and time-management skills to effectively manage a caseload of patients.
  13. Continuously strive to improve patient outcomes and promote patient satisfaction.
  14. Work with a sense of urgency and attention to detail to ensure timely and accurate completion of tasks and responsibilities.

Job Qualifications
  • Bachelor's Degree In Nursing: Amedisys Requires All Registered Nurse - Case Manager Applicants To Have A Minimum Of A Bachelor's Degree In Nursing From An Accredited Institution. This Degree Provides The Necessary Knowledge And Skills For The Role, Including Patient Assessment, Care Planning, And Coordination Of Services.

  • Current Rn License: Amedisys Requires All Applicants To Have A Valid And Current Registered Nurse (Rn) License. This License Demonstrates That The Individual Has Completed The Necessary Education And Training To Practice As A Nurse And Is Able To Provide Safe And Effective Care To Patients.

  • Case Management Experience: Amedisys Prefers Candidates With Previous Experience In Case Management. This Experience Shows That The Candidate Has The Necessary Skills To Coordinate And Manage Care For Patients With Complex Healthcare Needs.

  • Strong Communication And Interpersonal Skills: As A Case Manager, The Registered Nurse Will Be Working Closely With Patients, Families, And Other Healthcare Professionals. Therefore, Amedisys Looks For Candidates With Excellent Communication And Interpersonal Skills To Effectively Collaborate And Provide Quality Care.
