  1. Coordinate and manage patient care plans to ensure high-quality and compassionate care is provided.
  2. Utilize a strong clinical background to assess patient needs and develop appropriate care plans.
  3. Communicate effectively with patients, families, and healthcare team members to ensure all parties are informed and involved in the care process.
  4. Collaborate with other healthcare professionals to establish and implement best practices for patient care.

  9. Provide education and support to patients and their families regarding their care plans and any necessary lifestyle changes.
  10. Handle any potential conflicts or challenges that may arise during the course of patient care.
  11. Maintain a positive and professional attitude, promoting a welcoming and caring environment for patients.
  12. Participate in team meetings and contribute ideas for improving patient care and overall operations.
  13. Serve as a patient advocate, ensuring their needs and wishes are respected and addressed.
  14. Adhere to ethical and legal standards of practice.
  15. Participate in ongoing training and professional development opportunities.

Job Qualifications
  • Active Rn License: The Candidate Must Possess An Active And Unrestricted Rn License To Practice In The State Where The Job Is Located.

  • Minimum Of 2 Years Of Clinical Experience: Amedisys Requires A Minimum Of 2 Years Of Clinical Experience As An Rn, Preferably In A Home Health Or Hospice Setting.

  • Bsn Degree: Amedisys Values Nurses With A Bachelor Of Science In Nursing (Bsn) Degree, Although Candidates With An Associate Degree In Nursing (Adn) May Also Be Considered.

  • Case Management Experience: The Ideal Candidate Will Have Experience In Case Management, Including Coordinating Care And Collaborating With Other Healthcare Professionals.

  • Excellent Communication And Organizational Skills: As A Case Manager, The Candidate Must Possess Strong Communication And Organizational Skills To Effectively Manage Patient Care And Coordinate With Other Healthcare Team Members.
