Job Description

  1. Conduct comprehensive evaluations of patients to determine their specific occupational therapy needs.
  2. Develop individualized treatment plans based on evaluation results and patient goals.
  3. Implement evidence-based therapy techniques to address patients’ physical, cognitive, and emotional needs.
  4. Monitor and assess patient progress, adjusting treatment plans as needed.
  5. Collaborate with other healthcare professionals, such as physicians and nurses, to provide coordinated care to patients.
  6. Educate patients and their families on therapy exercises and techniques to facilitate carryover of treatment goals in the home setting.
  7. Maintain accurate and timely documentation of patient progress, treatment plans, and outcomes.
  8. Adhere to all ethical and legal standards, as well as organizational policies and procedures.
  9. Continuously strive to improve patient outcomes through professional development and staying up-to-date on best practices in occupational therapy.
  10. Demonstrate empathy, compassion, and respect for patients and their families.
  11. Foster a positive and supportive team dynamic by communicating effectively with colleagues and contributing to team meetings and discussions.
  12. Adhere to all safety protocols and infection control measures to ensure the health and wellbeing of patients and staff.
  13. Represent the organization in a professional manner at all times, both in and outside of the workplace.
  14. Act as an advocate for patients, promoting their rights and ensuring they receive the highest quality of care.
  15. Maintain confidentiality of all patient information in accordance with HIPAA regulations.

Job Qualifications
  • Bachelor's Or Master's Degree In Occupational Therapy From An Accredited Institution.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Families, And Interdisciplinary Teams.

  • Current State License Or Certification As An Occupational Therapist.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In Home Health Or Hospice Care.

  • Knowledge Of Evidence-Based Practices And Treatment Techniques In Occupational Therapy, With Experience In Developing And Implementing Personalized Treatment Plans.
