1. Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs to develop individualized care plans
2. Collaborate with interdisciplinary team members to provide high-quality and coordinated care to patients
3. Administer treatments and medications as prescribed by physicians and monitor patient response
4. Educate patients and their families on the palliative care process and provide resources for support and coping strategies
5. Conduct regular home visits and communicate updates to the team to ensure continuity of care
6. Monitor and document patient progress and adjust care plans as needed
7. Provide emotional support and counseling to patients and families during difficult times
8. Advocate for patients and collaborate with outside agencies to ensure comprehensive and holistic care
9. Stay up-to-date on current palliative care practices and participate in ongoing training and education opportunities

11. Adhere to ethical and legal standards in the provision of palliative care services
12. Communicate regularly with physicians and other team members to ensure coordinated and effective care for patients
