- Evaluate and assess the physical, emotional, and spiritual needs of hospice patients and their families.
- Develop and implement personalized care plans in collaboration with the interdisciplinary team.
- Conduct regular visits to patients to monitor their condition, manage symptoms, and provide support.
- Administer medication and treatments as prescribed by the attending physician.
- Educate patients and families about their illness, treatment options, and end-of-life care.
- Communicate effectively with patients, families, and the interdisciplinary team to ensure coordinated and comprehensive care.
- Maintain accurate and up-to-date patient records and documentation.
- Collaborate with other healthcare professionals to provide holistic care and address any concerns or issues.
- Participate in team meetings and contribute to the development of policies and procedures for hospice care.
- Stay up-to-date with current trends, practices, and research related to hospice care and end-of-life issues.
- Provide emotional support and counseling to patients and families during difficult times.
- Adhere to ethical and legal standards of practice and maintain confidentiality of patient information.
- Act as a liaison between patients, families, and the healthcare team to ensure all needs and preferences are met.
- Participate in on-call rotation and respond to urgent patient needs in a timely manner.
- Demonstrate a commitment to the mission and values of the organization and promote a positive work environment.
Strong Communication And Interpersonal Skills
Certification In Hospice And Palliative Care (Chpn) Preferred
Active Nurse Practitioner License In The State Of Practice
Minimum Of 2 Years Experience In Hospice Or Palliative Care
