
Medical Social Worker, MSW Hospice - Master's Degree

- Provide emotional support to patients and families facing end-of-life challenges.
- Utilize expertise and skills to provide individualized psychosocial assessments and develop care plans.
- Collaborate with interdisciplinary team to ensure holistic and comprehensive care for patients.
- Conduct home visits and provide counseling to patients and families as needed.
- Advocate for patients and families to ensure their needs and preferences are met.
- Offer guidance and resources for practical matters such as advance directives, financial planning, and long-term care options.
- Facilitate support groups and educational sessions for patients and families.
- Monitor patient and family coping mechanisms and provide interventions as needed.
- Maintain accurate and timely documentation of all patient interactions and progress.
- Stay up-to-date on current laws, regulations, and ethical standards related to hospice care and social work practice.
- Serve as a liaison between patients and families and community resources.
- Provide crisis intervention and bereavement support for patients and families.
- Collaborate with other healthcare providers and agencies to ensure continuity of care.
- Participate in interdisciplinary team meetings and provide input on patient care plans.
- Continuously evaluate and reassess patient and family needs and adjust care plans accordingly.
- Adhere to Amedisys policies and procedures, as well as professional standards and code of ethics.
- Maintain confidentiality and professionalism in all interactions with patients and families.
- Participate in ongoing education and training to enhance skills and knowledge.
- Act as an advocate for the hospice program and promote its mission and values.
- Ensure compliance with all regulatory and accreditation standards related to social work practice in hospice care.
Master's Degree In Social Work: A Medical Social Worker At Amedisys Must Have A Master's Degree In Social Work (Msw) From An Accredited University. This Advanced Degree Provides A Strong Foundation In Clinical Practice, Social Work Ethics, And Research Methods.
Hospice Experience: A Candidate For This Position Should Have Previous Experience Working In A Hospice Setting. This Experience Allows The Msw To Understand The Unique Needs Of Patients And Families Facing End-Of-Life Care, As Well As The Dynamics Of The Interdisciplinary Hospice Team.
Licensure And Certification: A Medical Social Worker At Amedisys Must Hold A Valid Clinical Social Work License In The State Where They Will Be Working. They Should Also Hold Certifications In Areas Such As Hospice And Palliative Care, Grief Counseling, And/Or End-Of-Life Care.
Strong Communication Skills: Effective Communication Is Essential For A Medical Social Worker In A Hospice Setting.
