Medical Social Worker, MSW Hospice - Master's Degree

4. Conduct home visits and provide counseling to patients and families as needed.

11. Serve as a liaison between patients and families and community resources.
12. Provide crisis intervention and bereavement support for patients and families.
13. Collaborate with other healthcare providers and agencies to ensure continuity of care.
14. Participate in interdisciplinary team meetings and provide input on patient care plans.
15. Continuously evaluate and reassess patient and family needs and adjust care plans accordingly.
16. Adhere to Amedisys policies and procedures, as well as professional standards and code of ethics.
17. Maintain confidentiality and professionalism in all interactions with patients and families.
18. Participate in ongoing education and training to enhance skills and knowledge.
19. Act as an advocate for the hospice program and promote its mission and values.
20. Ensure compliance with all regulatory and accreditation standards related to social work practice in hospice care.
