Job Description

Amedisys is seeking a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join our team on a PRN basis. As a Medical Social Worker, you will play a crucial role in providing support and resources to patients and their families during a challenging time. Your expertise in psychosocial assessment, counseling, and advocacy will be essential in helping our patients navigate their healthcare journey.   1. Conduct thorough psychosocial assessments of patients to determine their emotional, social, and financial needs.
  2. Provide counseling and emotional support to patients and their families, helping them cope with the challenges of their healthcare journey.
  3. Collaborate with interdisciplinary healthcare team to develop and implement comprehensive care plans that address patients' psychosocial needs.
  4. Advocate for patients' rights and assist them in accessing necessary resources and support services.
  5. Maintain accurate and up-to-date documentation of patient interactions and progress in accordance with company and regulatory guidelines.
  6. Remain current on relevant laws, regulations, and best practices in the social work field to ensure ethical and effective care for patients.
  7. Serve as a liaison between patients, their families, and other healthcare professionals to ensure continuity of care and effective communication.
  8. Participate in team meetings and case conferences to discuss patient progress and develop strategies for meeting their needs.
  9. Provide education and support to patients and their families on community resources and other available support services.
  10. Continuously evaluate and revise care plans as needed to meet the changing needs of patients and their families.
  11. Maintain confidentiality and adhere to all applicable laws and ethical standards in the handling of sensitive patient information.
  12. Collaborate with insurance companies and other third-party payers to obtain authorization for services and ensure timely reimbursement.
  13. Participate in professional development activities to enhance clinical skills and knowledge in the field of social work.
  14. Exhibit a high level of compassion, empathy, and sensitivity in all interactions with patients and their families.
  15. Adhere to company policies and procedures and promote a positive and professional work environment.

Job Qualifications
  • Knowledge And Understanding Of Medical Terminology, Disease Processes, And Treatment Options.

  • Licensed Master Of Social Work (Lmsw) Or Licensed Clinical Social Worker (Lcsw) In The State Of Employment.

  • Minimum Of 1-2 Years Of Experience Working As A Medical Social Worker In A Healthcare Setting.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Healthcare Team Members And Patients.

  • Ability To Provide Emotional Support And Counseling To Patients And Their Families Facing Medical Challenges.
