Job Description

As a Medical Social Worker, you will play a vital role in supporting our patients and their families by providing emotional and practical support during their healthcare journey. Our ideal candidate will be a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with a strong background in medical social work.   1. Provide emotional and practical support to patients and their families during their healthcare journey.
  2. Conduct thorough assessments of patients' psychosocial needs and develop individualized care plans.
  3. Collaborate with interdisciplinary team members to ensure the best possible care for patients.
  4. Educate patients and their families on available resources and support services.
  5. Advocate for patients' rights and assist with navigating the healthcare system.
  6. Utilize evidence-based interventions to address patients' emotional and social needs.
  7. Maintain accurate and up-to-date documentation of patient interactions and progress.
  8. Uphold professional and ethical standards in accordance with state and federal regulations.
  9. Participate in team meetings and case conferences to discuss patient care and develop treatment plans.
  10. Continuously assess and address any potential barriers to care for patients.
  11. Provide crisis intervention and support during times of emotional distress.
  12. Collaborate with community organizations to provide additional support and resources for patients.
  13. Stay current on industry developments and best practices in medical social work.
  14. Maintain confidentiality and respect the privacy of patients and their families.
  15. Demonstrate empathy, compassion, and cultural sensitivity in all interactions with patients and their families.

Job Qualifications

  • Master's Degree In Social Work: A Qualified Candidate For A Medical Social Worker Position At Amedisys Should Possess A Master's Degree In Social Work From An Accredited University. This Education Provides A Strong Foundation In Social Work Theory And Practice, Which Is Essential For Providing Effective Medical Social Services.

  • Licensure: Amedisys Typically Requires Candidates To Hold Either A Licensed Master Social Worker (Lmsw) Or Licensed Clinical Social Worker (Lcsw) License. This Knowledge Is Crucial For Helping Patients Navigate The Complex Healthcare System And Access Necessary Resources.

  • Empathy And Communication Skills: A Successful Medical Social Worker Must Possess Strong Empathy And Communication Skills To Effectively Connect With Patients And Their Families. This Includes Active Listening, Effective Communication, And The Ability To Build Rapport And Trust With Patients From Diverse Backgrounds.
