  1. Provide emotional support and counseling to patients and their families during difficult and challenging times.
  2. Conduct comprehensive assessments of patients' social and emotional needs to develop individualized care plans.
  3. Collaborate with other members of the healthcare team to coordinate and provide holistic care for patients.
  4. Assist patients and families in navigating and accessing community resources and support services.
  5. Advocate for patients' needs and rights, ensuring they receive appropriate and timely care.
  6. Educate patients and families about their medical conditions and treatment options.
  7. Maintain accurate and detailed documentation of patient interactions and progress.
  8. Monitor and evaluate patients' mental and emotional well-being and provide appropriate interventions as needed.
  9. Collaborate with other healthcare professionals to address complex social and emotional issues that may impact patient care.
  10. Stay updated on current healthcare policies, regulations, and best practices related to medical social work.
  11. Act as a liaison between patients, families, and healthcare facilities to ensure effective communication and coordination of care.
  12. Continuously communicate and collaborate with patients and their families to address any concerns or challenges they may be facing.
  13. Maintain confidentiality and adhere to ethical standards in all patient interactions and decision-making.
  14. Participate in training and development opportunities to enhance knowledge and skills in the field of medical social work.

Job Qualifications
  • Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Families, And Interdisciplinary Healthcare Teams.

  • Master's Degree In Social Work From An Accredited Program.

  • Current Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of Employment.

  • Minimum Of 2 Years Experience In Medical Social Work, Preferably In A Home Health Or Hospice Setting.

  • Strong Understanding Of Medical Terminology And Healthcare Systems, As Well As Knowledge Of Community Resources And Benefits Programs.
