- Collaborate with interdisciplinary team members, including physicians, nurses, therapists, and other healthcare professionals, to ensure comprehensive and coordinated care for patients.
- Provide timely and appropriate interventions, counseling, and support to patients and their families to address psychosocial, emotional, and financial challenges.
- Educate patients and their families about available resources, community support services, and healthcare options to enhance their overall well-being.
- Advocate for patients' rights and help them navigate the healthcare system to access necessary services and benefits.
- Maintain accurate and up-to-date documentation of all patient interactions, assessments, interventions, and progress in accordance with company and regulatory standards.
- Collaborate with external agencies and community organizations to coordinate services and facilitate referrals for patients' ongoing needs.
- Participate in interdisciplinary team meetings and case conferences to discuss patient progress, challenges, and updates.
- Keep abreast of changes in healthcare policies, regulations, and community resources to provide the most current and relevant support to patients.
- Uphold ethical and professional standards in all interactions with patients, families, and colleagues.
Bachelor's Or Master's Degree In Social Work: Amedisys Requires All Medical Social Workers To Have A Minimum Of A Bachelor's Degree In Social Work. Preference Will Be Given To Candidates With A Master's Degree In Social Work.
State Licensure And Certification: Candidates Must Hold A Current State License And Certification As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw). In Addition, Candidates Must Maintain All Necessary Certifications And Licensure Throughout Their Employment With Amedisys.
Experience In Healthcare Setting: A Minimum Of 2 Years Of Experience Working In A Healthcare Setting, Preferably In Home Health Or Hospice Care, Is Required For This Position. Candidates Must Have A Strong Understanding Of Medical Terminology And The Ability To Work Collaboratively With Other Healthcare Professionals.
Strong Communication And Interpersonal Skills: Medical Social Workers At Amedisys Must Possess Excellent Communication And Interpersonal Skills. They Must Be Able To Effectively Communicate With Patients, Families, And Other Healthcare Professionals In A Compassionate And Empathetic Manner.
Knowledge Of Social Work Ethics And Standards: Amedisys Expects All Medical Social Workers To Adhere To The Highest Ethical Standards In Their Practice. Candidates Must Have A Strong Understanding Of Social Work Ethics And Be Able To Apply These Principles In Their Daily Work With Patients And Families.
