6. Maintain accurate and timely documentation of all patient interactions and interventions.
7. Monitor and evaluate the effectiveness of the care plan and make necessary modifications as needed.
8. Participate in team meetings and case conferences to discuss patient progress and develop effective treatment plans.
9. Adhere to all ethical and professional standards in the delivery of care.
10. Keep updated on current trends and developments in the field of medical social work and implement best practices in patient care.
11. Maintain confidentiality and respect the rights and dignity of patients and their families at all times.
12. Collaborate with community organizations and agencies to facilitate access to additional resources for patients and their families.
13. Handle crisis situations and provide appropriate interventions to ensure the safety and well-being of patients.
14. Communicate effectively with patients, families, and team members to ensure a positive and supportive care environment.
