  1. Provide compassionate and comprehensive care to patients in their homes.
  2. Conduct thorough assessments of patients' psychosocial and emotional needs.
  3. Collaborate with interdisciplinary team to develop and implement individualized care plans.
  4. Advocate for patients and their families to ensure their needs are met within the healthcare system.
  5. Educate patients and their families on available resources and support services.
  6. Provide counseling and emotional support to patients and families.
  7. Monitor patient progress and make necessary adjustments to care plans.
  8. Maintain accurate and up-to-date documentation of patient care.
  9. Communicate effectively with healthcare team and external resources.
  10. Participate in case conferences and contribute to patient care discussions.
  11. Stay up-to-date on current healthcare policies, regulations and resources.
  12. Adhere to ethical and professional standards of practice.
  13. Collaborate with community organizations to provide additional support to patients.
  14. Provide crisis intervention and referrals as needed.
  15. Continuously evaluate and improve patient care processes.

Job Qualifications
  • Master's Degree In Social Work From An Accredited University.

  • Current Licensure As A Licensed Clinical Social Worker (Lcsw) Or Licensed Master Social Worker (Lmsw) In The State Of Employment.

  • Minimum Of 2 Years Of Clinical Social Work Experience In A Healthcare Setting, Preferably In Hospice Or Home Health.

  • Knowledge Of Medical Terminology And Understanding Of Healthcare Systems And Regulations.

  • Strong Communication And Collaboration Skills, With The Ability To Work Effectively With Interdisciplinary Teams And Diverse Patient Populations.
