  1. Provide emotional and psychological support to patients and their families during their healthcare journey.
  2. Assess patient and family needs and develop a comprehensive care plan to address their social and emotional well-being.
  3. Collaborate with interdisciplinary team members to ensure continuity and coordination of care for patients.
  4. Conduct thorough psychosocial assessments to identify any barriers to care and make appropriate referrals to community resources.
  5. Advocate for patients and families to ensure their needs and preferences are respected and addressed.
  6. Educate patients and families on available resources and services to help them cope with their healthcare needs.
  7. Document all interactions and interventions in a timely and accurate manner.
  8. Provide crisis intervention as needed and make appropriate referrals for mental health services.
  9. Participate in care planning meetings and provide input on the social and emotional aspects of patient care.
  10. Adhere to all ethical and legal standards in the provision of care and maintain patient confidentiality.
  11. Continuously educate yourself on best practices and stay updated on changes in the healthcare industry.
  12. Attend training and professional development opportunities to enhance your knowledge and skills.
  13. Build and maintain positive relationships with patients, families, and community resources.
  14. Collaborate with insurance companies and other payers to ensure appropriate coverage for patient care.
  15. Foster a positive and compassionate work environment by demonstrating empathy, respect, and professionalism towards patients and colleagues.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited Program.

  • Current Licensure Or Certification As A Licensed Clinical Social Worker (Lcsw) In The State Of Employment.

  • Minimum Of 2 Years Of Experience In A Medical Social Work Role, Preferably In A Home Health Or Hospice Setting.

  • Knowledge Of Medical Terminology, Healthcare Systems, And Community Resources.

  • Strong Communication And Interpersonal Skills, With The Ability To Provide Compassionate Care And Support To Patients And Their Families.
