  1. Provide emotional support and counseling to patients and their families during their healthcare journey.
  2. Utilize social work principles and techniques to assess patients' needs and develop individualized care plans.
  3. Collaborate with interdisciplinary team members to establish and implement appropriate treatment plans.
  4. Conduct psychosocial assessments and identify potential barriers to care.
  5. Connect patients and families with community resources and support systems.
  6. Advocate for patients' rights and ensure they receive appropriate care and services.
  7. Maintain accurate and timely documentation of patient interactions and progress.
  8. Participate in team meetings and case conferences to discuss patient care and coordinate services.
  9. Continuously evaluate and monitor patients' progress and adjust treatment plans as needed.
  10. Educate patients and families about the importance of mental health and self-care.
  11. Provide crisis intervention services as needed.
  12. Collaborate with insurance companies and other third-party payers to ensure proper coverage for patients.
  13. Keep up-to-date with healthcare laws, regulations, and policies related to social work.
  14. Attend trainings and conferences to enhance knowledge and skills.
  15. Uphold ethical standards and maintain confidentiality of patient information.
  16. Foster a positive and supportive work environment.
  17. Communicate effectively and professionally with patients, families, and colleagues.
  18. Continuously seek opportunities for personal and professional growth.
  19. Adhere to all organization policies and procedures.
  20. Maintain a positive and compassionate attitude towards patients and their families.

Job Qualifications
  • A Master's Degree In Social Work (Msw) From An Accredited University.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Effectively With Patients, Families, And Interdisciplinary Healthcare Teams.

  • Minimum Of 2-3 Years Of Experience In A Medical Or Healthcare Setting, Preferably In Home Health Care.

  • Active State Licensure As A Licensed Clinical Social Worker (Lcsw) Or Clinical Social Worker (Lcsw) Preferred.

  • Strong Knowledge Of Medical Terminology, Healthcare Laws And Regulations, And Insurance Processes.
