1. Provide compassionate and personalized nursing care to patients in their homes.
2. Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients.
3. Monitor and assess patients' health status, and communicate any changes or concerns to the team.
4. Administer medications and treatments as prescribed by the physician.
5. Educate patients and their families on disease management, medication management, and other health-related topics.
6. Document all patient care activities accurately and timely.
7. Comply with all state and federal regulations, as well as company policies and procedures.
8. Utilize critical thinking and problem-solving skills to ensure the highest quality of care for patients.
9. Build and maintain positive and professional relationships with patients and their families.
10. Attend and actively participate in team meetings, case conferences, and in-service education.
11. Continuously strive to improve clinical skills and knowledge through ongoing education and training.
12. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
13. Demonstrate a commitment to the mission and values of the company.
14. Perform other duties as assigned by the supervisor or manager.
