  1. Provide compassionate and high-quality nursing care to patients in their homes.

  3. Administer medications and treatments according to physician orders.

  5. Educate patients and their families on disease management, medication administration, and home care techniques.
  6. Monitor and document patients' progress and report any changes or concerns to the physician.
  7. Ensure all equipment and supplies are properly maintained and stocked.
  8. Adhere to all company policies, procedures, and regulations.
  9. Maintain accurate and timely documentation of patient care activities.
  10. Provide emotional support and comfort to patients and their families.
  11. Participate in ongoing training and education to enhance skills and knowledge.
  12. Serve as an advocate for patients and their needs.
  13. Maintain confidentiality and professionalism at all times.
  14. Adhere to infection control and safety protocols.
  15. Provide excellent customer service and maintain positive relationships with patients and their families.
  16. Demonstrate empathy, patience, and understanding towards patients.
  17. Attend and actively participate in team meetings and case conferences.
  18. Continuously strive to improve patient outcomes and satisfaction.
  19. Maintain accurate and up-to-date knowledge of nursing practices and procedures.

Job Qualifications
  • Valid Lpn Licensure: The Candidate Must Possess A Valid And Current Licensed Practical Nurse (Lpn) License From The State In Which They Will Be Providing Home Health Services.

  • Previous Home Health Experience: A Minimum Of One Year Of Experience Working In A Home Health Setting Is Preferred For This Role. This Ensures That The Candidate Is Familiar With The Unique Challenges And Responsibilities Of Caring For Patients In Their Homes.

  • Strong Clinical Skills: The Ideal Candidate Should Have A Strong Foundation In Nursing Skills And Knowledge, With The Ability To Provide Skilled Nursing Care To Patients In A Home Health Setting.

  • Excellent Communication And Interpersonal Skills: As A Lpn Home Health Nurse, The Candidate Will Be Working Closely With Patients, Their Families, And Other Healthcare Professionals. Therefore, Strong Communication And Interpersonal Skills Are Essential For Building Rapport And Providing Quality Care.

  • Ability To Work Independently: A Lpn Home Health Nurse Must Be Able To Work Independently And Make Decisions Without Direct Supervision. This Requires Strong Critical Thinking Skills And The Ability To Adapt To Changing Situations.
