1. Provide high-quality and compassionate care to patients in their homes.
2. Administer medications and treatments as prescribed by the physician.
3. Monitor and report changes in patient's condition to the physician and other members of the healthcare team.
4. Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
5. Educate patients and their families on disease management, medication regimen, and self-care techniques.
6. Document all care and interactions accurately and timely in the patient's medical record.
7. Ensure compliance with all state and federal regulations and company policies.
8. Maintain a safe and clean environment for patients and adhere to infection control protocols.
9. Communicate effectively with patients, families, and other healthcare team members.
10. Participate in ongoing education and training to enhance knowledge and skills.
11. Adhere to ethical and professional standards of nursing practice.
12. Promote a positive work environment by demonstrating teamwork and professionalism.
13. Follow established procedures and protocols for emergency situations.
14. Maintain confidentiality of patient information.
15. Serve as an advocate for patients and their families to ensure their needs are met.
16. May be required to travel to different patient homes throughout the day.
17. Participate in performance improvement activities to enhance the quality of care provided.
