Amedisys is seeking a highly organized and compassionate LPN Clinical Patient Services Coordinator to join our team.   1. Coordinate and manage patient services for home health and hospice care.
  2. Ensure timely and accurate scheduling of patient appointments and visits.
  3. Communicate with patients, families, and healthcare providers to provide updates and address any concerns.
  4. Conduct patient assessments and develop care plans in collaboration with the interdisciplinary team.
  5. Monitor and document patient progress and report any changes to the appropriate healthcare professionals.
  6. Administer medications and treatments as prescribed by the physician.
  7. Maintain accurate and organized patient records.
  8. Educate patients and their families on disease management, medication administration, and other aspects of care.
  9. Collaborate with insurance companies to obtain necessary authorizations for services.
  10. Adhere to all regulatory and compliance guidelines.
  11. Participate in quality improvement initiatives and continuously seek ways to improve patient care.
  12. Serve as a liaison between patients, families, and the healthcare team.
  13. Provide emotional support and compassionate care to patients and their families.
  14. Assist with discharge planning and coordination of services after a patient's stay.
  15. Keep up-to-date with industry developments and best practices in home health and hospice care.

Job Qualifications
  • Current Lpn Licensure: The Candidate Must Hold A Valid And Active Lpn License In The State Where They Will Be Working.

  • Previous Clinical Experience: A Minimum Of 2 Years Of Experience As An Lpn In A Clinical Setting Is Required, Preferably In A Home Health Or Hospice Setting.

  • Strong Communication Skills: The Candidate Must Possess Excellent Verbal And Written Communication Skills, As They Will Be Responsible For Communicating With Patients, Families, And Other Healthcare Professionals.

  • Organizational Skills: As A Patient Services Coordinator, The Candidate Must Be Highly Organized And Able To Manage Multiple Tasks And Priorities Effectively.

  • Knowledge Of Home Health Regulations: The Candidate Should Have A Strong Understanding Of Home Health Regulations And Guidelines, Including Medicare And Medicaid Guidelines, To Ensure Compliance In Patient Care.
