
LPN Clinical Patient Services Coordinator

- Coordinate and manage patient services for home health and hospice care.
- Ensure timely and accurate scheduling of patient appointments and visits.
- Communicate with patients, families, and healthcare providers to provide updates and address any concerns.
- Conduct patient assessments and develop care plans in collaboration with the interdisciplinary team.
- Monitor and document patient progress and report any changes to the appropriate healthcare professionals.
- Administer medications and treatments as prescribed by the physician.
- Maintain accurate and organized patient records.
- Educate patients and their families on disease management, medication administration, and other aspects of care.
- Collaborate with insurance companies to obtain necessary authorizations for services.
- Adhere to all regulatory and compliance guidelines.
- Participate in quality improvement initiatives and continuously seek ways to improve patient care.
- Serve as a liaison between patients, families, and the healthcare team.
- Provide emotional support and compassionate care to patients and their families.
- Assist with discharge planning and coordination of services after a patient's stay.
- Keep up-to-date with industry developments and best practices in home health and hospice care.
Current Lpn Licensure: The Candidate Must Hold A Valid And Active Lpn License In The State Where They Will Be Working.
Previous Clinical Experience: A Minimum Of 2 Years Of Experience As An Lpn In A Clinical Setting Is Required, Preferably In A Home Health Or Hospice Setting.
Strong Communication Skills: The Candidate Must Possess Excellent Verbal And Written Communication Skills, As They Will Be Responsible For Communicating With Patients, Families, And Other Healthcare Professionals.
Organizational Skills: As A Patient Services Coordinator, The Candidate Must Be Highly Organized And Able To Manage Multiple Tasks And Priorities Effectively.
Knowledge Of Home Health Regulations: The Candidate Should Have A Strong Understanding Of Home Health Regulations And Guidelines, Including Medicare And Medicaid Guidelines, To Ensure Compliance In Patient Care.
