- Provide high-quality, compassionate care to patients in their homes.
- Administer medications and treatments as prescribed by physicians.
- Monitor and assess patient’s health status and report any changes to the appropriate healthcare team members.
- Educate patients and their families on proper care and management of their health conditions.
- Maintain accurate and up-to-date patient records.
- Collaborate with other healthcare professionals to develop and implement individualized care plans.
- Adhere to all nursing standards and protocols.
- Ensure patient safety and comfort at all times.
- Communicate effectively with patients, families, and colleagues.
- Participate in ongoing training and development to maintain and improve nursing skills.
- Adhere to all applicable laws, regulations, and ethical standards.

- Act as an advocate for patients and their families.
- Continuously evaluate and improve the quality of patient care.
State Licensure: A Current And Valid License As A Licensed Practical Nurse (Lpn) In The State Where The Job Is Located.
Education: Completion Of An Accredited Practical Nursing Program And/Or Equivalent Education And Experience.
Experience: Minimum Of One Year Of Clinical Experience As An Lpn In A Healthcare Setting, Preferably With Home Health Experience.
Knowledge And Skills: Demonstrated Knowledge Of Nursing Principles, Practices, And Techniques As Well As The Ability To Effectively Communicate And Collaborate With Patients, Families, And The Healthcare Team.
Certifications: Current Cpr Certification And Other Required Certifications, Such As Iv Therapy And Wound Care, As Outlined By State Regulations And Amedisys Policies.
