  1. Conduct comprehensive assessments of patients' social, emotional, and financial needs to develop individualized care plans.
  2. Provide counseling and support to patients and their families to enhance coping skills and improve overall well-being.
  3. Collaborate with the interdisciplinary team to develop and implement patient care plans that address social and emotional needs.
  4. Communicate effectively with patients, their families, and healthcare team members to ensure coordinated and quality care.
  5. Assist patients and their families in navigating community resources and support services.
  6. Advocate for patients' rights and needs, ensuring their voices are heard and respected.
  7. Continuously evaluate and reassess patients' needs and adjust care plans accordingly.
  8. Document all patient interactions, assessments, and care plans accurately and timely.
  9. Adhere to all ethical and professional standards of practice and maintain patient confidentiality at all times.
  10. Participate in team meetings, trainings, and other opportunities for professional development.
  11. Stay updated on industry trends and best practices in social work to enhance service delivery.
  12. Provide guidance and support to less experienced social workers and other team members.
  13. Comply with all organizational policies, procedures, and regulations to ensure compliance with legal and ethical standards.
  14. Continuously strive to improve the quality and efficiency of patient care by identifying and addressing any barriers or challenges.
  15. Serve as a positive and professional representative of Amedisys in all interactions with patients, families, and the community.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Program

  • Current State License As A Licensed Clinical Social Worker (Lcsw)

  • Minimum Of Two Years Of Experience In A Healthcare Setting, Preferably In Hospice Or Home Health

  • Knowledge Of Medical Terminology And Healthcare Systems, As Well As Understanding Of Psychosocial Factors Impacting Patient Care

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Effectively Advocate For Patients' Needs
