  1. Provide high-quality and compassionate medical social services to patients in accordance with company standards and policies.
  2. Conduct thorough assessments of patients' social, emotional, and financial needs to determine appropriate care plans.
  3. Collaborate with interdisciplinary team members to develop and implement individualized care plans for patients.
  4. Educate patients and their families on available community resources and assist with making appropriate referrals.
  5. Advocate for patients' rights and ensure their needs are being met.
  6. Maintain accurate and timely documentation of patient assessments, care plans, and progress notes.
  7. Comply with all state and federal regulations related to medical social work.
  8. Participate in interdisciplinary team meetings to discuss patient care and provide input on social work interventions.
  9. Continuously seek out opportunities for professional development and stay updated on best practices in medical social work.
  10. Uphold the company's values of integrity, teamwork, and excellence in all interactions with patients, colleagues, and the community.
  11. Adhere to ethical standards and maintain patient confidentiality at all times.
  12. Collaborate with insurance companies and other third-party payers to ensure appropriate coverage and reimbursement for services.
  13. Communicate effectively with patients, families, and caregivers to build trusting relationships and address any concerns or challenges.
  14. Participate in company initiatives and projects to improve the delivery of care and enhance patient outcomes.
  15. Maintain a valid social work license and adhere to all requirements for license renewal.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Program.

  • Active And Unrestricted State License To Practice As A Social Worker.

  • Minimum Of Two Years Of Experience In A Healthcare Setting, Preferably In A Home Health Or Hospice Setting.

  • Knowledge Of Medical Terminology, Disease Processes, And Treatment Plans.

  • Ability To Provide Compassionate And Culturally Sensitive Care To Patients And Their Families.
