In this role, you will serve as a vital link between our palliative care team and our patients, their families, and other healthcare providers.   1. Develop and maintain relationships with patients, families, and healthcare providers to effectively communicate the benefits and services of our palliative care program.
  2. Serve as a liaison between our palliative care team and other healthcare providers, ensuring seamless communication and coordination of care.
  3. Conduct thorough assessments of patient and family needs, goals, and preferences to develop individualized care plans.
  4. Educate patients and families on palliative care principles, including pain management, symptom control, and emotional support.
  5. Collaborate with the interdisciplinary team to ensure the delivery of high-quality, compassionate care to patients and families.
  6. Provide ongoing support and guidance to patients and families throughout the palliative care journey.
  7. Coordinate and facilitate family meetings and care conferences to discuss treatment plans and goals of care.
  8. Maintain accurate and up-to-date documentation of all patient interactions, assessments, and care plans.
  9. Participate in community outreach initiatives to promote awareness and understanding of palliative care services.
  10. Continuously evaluate and improve the palliative care program through data analysis and feedback from patients, families, and healthcare providers.

Job Qualifications
  • Knowledge And Experience In Palliative Care: The Ideal Candidate Should Possess A Strong Understanding Of Palliative Care Principles And Practices, As Well As Experience Working In A Palliative Care Setting.

  • Communication And Interpersonal Skills: A Successful Liaison In Palliative Care Must Have Excellent Communication Skills, Including The Ability To Effectively Collaborate With Interdisciplinary Teams, Patients And Their Families, And Other Healthcare Providers.

  • Compassion And Empathy: Palliative Care Involves Caring For Patients Who Are Facing Serious Or Terminal Illnesses. A Liaison Must Have A Compassionate And Empathetic Approach To Provide Emotional Support And Comfort To Patients And Their Families.

  • Organizational And Time Management Skills: Palliative Care Liaisons Are Responsible For Managing A Caseload Of Patients, Coordinating Care, And Maintaining Documentation. Therefore, Strong Organizational And Time Management Skills Are Essential For Success In This Role.

  • Education And Training: Amedisys May Require Candidates To Have A Bachelor's Or Master's Degree In A Healthcare-Related Field. Additional Training Or Certification In Palliative Care May Also Be Preferred.
