- Serve as a primary point of contact for patients and families participating in the Palliative Care program.
- Conduct initial assessments and ongoing evaluations of patients to determine their eligibility for the program.
- Collaborate with interdisciplinary team members to develop individualized care plans for patients.
- Communicate regularly with patients, families, and healthcare providers to ensure effective coordination of care.
- Educate patients and families on the benefits and goals of the Palliative Care program.
- Facilitate communication and decision-making between patients, families, and healthcare providers.
- Advocate for patients and their families to ensure their needs and preferences are being met.
- Maintain accurate and timely documentation of patient care and progress.
- Continuously evaluate and make recommendations for improvements to the Palliative Care program.
- Stay up-to-date on the latest developments and best practices in palliative care to provide the highest quality of care for patients.
- Adhere to all regulatory and compliance guidelines at the local, state, and federal levels.
- Participate in team meetings, trainings, and other professional development opportunities.
- Represent Amedisys and the Palliative Care program in a professional and compassionate manner.
- Demonstrate a commitment to providing compassionate and high-quality end-of-life care to patients and their families.
- Maintain confidentiality and uphold ethical standards in all interactions with patients and their families.
Knowledge And Experience In Palliative Care: The Ideal Candidate For A Liaison - Palliative Care Job At Amedisys Should Have A Strong Understanding Of Palliative Care Principles, Practices, And Regulations. This Includes Knowledge Of Pain Management, Symptom Control, And End-Of-Life Care.
Excellent Communication Skills: As A Liaison, The Candidate Must Be Able To Effectively Communicate With Patients, Families, And Healthcare Professionals. This Includes Having Strong Verbal And Written Communication Skills, As Well As The Ability To Listen And Empathize With Others.
