
Liaison - Palliative Care

In this role, you will act as a liaison between our patients, their families, and our interdisciplinary care team, ensuring seamless and comprehensive care. - Serve as a point of contact for patients and their families, providing support and guidance throughout the palliative care process.
- Collaborate with the interdisciplinary care team to ensure all aspects of patient care are addressed and communicated effectively.
- Conduct initial assessments of patients and their families to determine eligibility for the palliative care program.
- Educate patients and families on the goals and benefits of palliative care, as well as available resources and services.
- Coordinate and schedule appointments, procedures, and consultations for patients.
- Maintain accurate and up-to-date patient records, including progress notes and treatment plans.
- Communicate effectively with physicians, nurses, social workers, and other healthcare professionals to facilitate coordinated care.
- Act as an advocate for patients and their families, addressing any concerns or issues that may arise.
- Participate in team meetings and contribute insights and suggestions for improving patient care.
- Stay informed about current trends and developments in palliative care to continually improve patient outcomes.
- Adhere to all relevant laws, regulations, and ethical standards in the provision of care.
- Uphold the values and mission of Amedisys, promoting a culture of compassion, respect, and excellence.
- Continuously evaluate and improve processes to enhance the efficiency and effectiveness of the palliative care program.
- Provide emotional support and guidance to patients and families during difficult times.
- Maintain confidentiality and protect patient privacy at all times.
Experience In Palliative Care: The Ideal Candidate Should Have A Minimum Of 2-3 Years Of Experience Working In Palliative Care, Preferably In A Hospice Or Home Health Setting. This Experience Should Include Working With Patients And Families To Provide Comprehensive End-Of-Life Care.
Strong Communication Skills: As A Liaison, The Candidate Will Be Responsible For Communicating With Patients, Families, And Healthcare Providers. Therefore, Excellent Verbal And Written Communication Skills Are Essential To Effectively Convey Information And Build Relationships.
Knowledge Of Hospice Regulations: A Thorough Understanding Of Hospice Regulations And Medicare Guidelines Is Crucial For This Role. The Candidate Should Be Familiar With The Documentation And Coding Requirements For Hospice Services And Be Able To Ensure Compliance With All Regulations.
Clinical Background: While This Position Is Primarily Focused On Communication And Coordination, A Clinical Background In Nursing Or Social Work Is Preferred. This Will Provide A Strong Foundation For Understanding The Needs Of Palliative Care Patients And Collaborating With The Interdisciplinary Team.
Empathy And Compassion: Palliative Care Can Be Emotionally Challenging For Patients And Families, And The Liaison Should Possess Empathy, Compassion, And The Ability To Provide Emotional Support. The Candidate Should Also Be Able To Handle Difficult Conversations With Sensitivity And Professionalism.
