Job Description

We are currently seeking a compassionate and organized Intake Coordinator to join our team on a PRN (as needed) basis. In this non-clinical role, you will be responsible for coordinating patient admissions and ensuring a smooth transition into our care.   1. Coordinate patient admissions: Responsible for managing the process of admitting new patients into our care, including gathering necessary information and documentation, communicating with healthcare providers, and ensuring all paperwork is completed accurately and in a timely manner.
  2. Ensure smooth transition into care: Work closely with the patient and their family to ensure a smooth transition into our care, addressing any concerns or questions they may have. This includes scheduling appointments, arranging transportation, and providing necessary information about our services.
  3. Maintain accurate records: Keep detailed and accurate records of all patient admissions, including patient information, medical history, and insurance details. This information should be organized and easily accessible for future reference.
  4. Communicate effectively: Serve as the main point of contact for patients and their families during the intake process, and communicate effectively with other members of the healthcare team. This includes relaying important information and addressing any issues that may arise.
  5. Adhere to policies and procedures: Follow all company policies and procedures, as well as federal and state regulations, to ensure the highest level of patient care and confidentiality.
  6. Provide exceptional customer service: Greet patients and their families with a warm and welcoming attitude, and provide exceptional customer service throughout the intake process. This includes addressing any concerns or complaints in a timely and professional manner.
  7. Collaborate with team members: Work closely with other members of the healthcare team, including nurses, doctors, and administrative staff, to ensure a seamless intake process and optimal patient care.
  8. Maintain confidentiality: Handle all patient information with the utmost confidentiality, adhering to HIPAA regulations and company policies at all times.
  9. Attention to detail: Pay close attention to detail when reviewing patient information and completing paperwork, ensuring accuracy and completeness.
  10. Uphold company values: Embody the values and mission of Amedisys in all interactions with patients, their families, and team members.

Job Qualifications
  • Strong Communication And Organizational Skills.

  • Bachelor's Degree In A Related Field Such As Healthcare Administration, Social Work, Or Psychology.

  • Knowledge Of Medical Terminology And Healthcare Processes.

  • Previous Experience In A Healthcare Setting, Preferably In A Coordinating Or Administrative Role.
