  1. Conduct comprehensive psychosocial assessments of patients and their families to identify their needs and develop appropriate care plans.
  2. Collaborate with interdisciplinary healthcare team members to ensure comprehensive and holistic care for patients.
  3. Provide emotional support and counseling to patients and their families during difficult times, including end-of-life care.
  4. Connect patients and families with community resources and support services to enhance their quality of life.
  5. Advocate for patients and their families to ensure their needs are met and their voices are heard.
  6. Maintain accurate and timely documentation of all patient interactions and interventions.
  7. Participate in team meetings and provide recommendations for patient care.
  8. Stay up-to-date on current social work practices and regulations related to healthcare.
  9. Educate patients and families on coping mechanisms and self-care strategies.
  10. Collaborate with insurance providers to ensure coverage for necessary services.
  11. Provide crisis intervention and support in emergency situations.
  12. Conduct home visits and assess the safety and well-being of patients in their environment.
  13. Maintain confidentiality and adhere to ethical standards in all interactions with patients and their families.
  14. Communicate effectively with patients, families, and healthcare team members to ensure a coordinated and seamless care experience.
  15. Participate in quality improvement initiatives to enhance the overall patient experience.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited University.

  • Minimum Of 2 Years Of Experience In A Medical Social Work Role, Preferably In A Home Health Or Hospice Setting.

  • Active State Licensure As A Licensed Clinical Social Worker (Lcsw).

  • Knowledge Of Medicare/Medicaid Regulations And Documentation Requirements.

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Healthcare Teams And Provide Compassionate Support To Patients And Their Families.
