1. Provide compassionate and specialized care to hospice patients and their families during their end-of-life journey.
2. Conduct assessments and develop individualized care plans for patients in collaboration with the hospice team.
3. Administer medications and treatments as prescribed, and monitor patient response and progress.
4. Communicate effectively and sensitively with patients and their families, providing emotional support and addressing any concerns or questions.
5. Collaborate with the interdisciplinary hospice team to coordinate and implement comprehensive care for patients.
6. Perform physical examinations, order and interpret diagnostic tests, and make appropriate referrals as needed.
7. Maintain accurate and timely documentation of patient care, including progress notes and care plans.
8. Educate patients and their families about their illness, treatment options, and end-of-life care.
9. Stay current with industry standards and best practices in hospice care through ongoing education and training.
10. Adhere to all company policies and procedures, as well as state and federal regulations, to ensure quality care and patient safety.
11. Participate in quality improvement activities and assist in developing and implementing strategies to improve patient outcomes.
12. Serve as a mentor and resource for other hospice team members, sharing knowledge and expertise to enhance patient care.
13. Continuously assess and evaluate the effectiveness of care and make adjustments as necessary to ensure the best possible outcomes for patients.
14. Maintain a professional and positive attitude, representing the company and its values to patients, families, and the community.
15. Collaborate with community resources and organizations to expand and improve hospice services.
