1. Provide compassionate and empathetic care to patients and their families during their end-of-life journey.
2. Collaborate with interdisciplinary team members to develop and implement individualized care plans.
3. Communicate effectively with patients, families, and team members to ensure understanding and coordination of care.
4. Conduct comprehensive assessments and evaluations of patients’ physical, emotional, and psychosocial needs.
5. Administer medications, treatments, and procedures according to established protocols and within scope of practice.
6. Monitor and document patient progress, symptoms, and changes in condition.
7. Educate patients and families on disease processes, symptom management, and end-of-life care.
8. Provide emotional support and counseling to patients and families as needed.
9. Advocate for patients’ rights and preferences in regards to their care.
10. Collaborate with hospice medical director in managing complex cases and making treatment decisions.
11. Attend and participate in interdisciplinary team meetings and case conferences.
12. Stay current on hospice care best practices, regulations, and policies.
13. Maintain accurate and timely documentation of patient care.
14. Adhere to all ethical and legal standards of practice.
15. Maintain a positive and professional attitude while handling emotional situations with sensitivity and grace.
16. Serve as a resource and mentor to other team members.
17. Participate in quality improvement initiatives to ensure high-quality and efficient care.
