- Conduct initial patient assessments and develop individualized care plans based on their physical, emotional, and spiritual needs
- Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care for patients
- Provide ongoing pain and symptom management for patients with life-limiting illnesses
- Educate patients and families about the hospice philosophy and services, as well as their rights and options
- Communicate regularly with patients' primary care physicians and specialists to ensure continuity of care
- Document all patient care activities accurately and thoroughly in electronic medical records
- Offer emotional and spiritual support to patients and families during the end-of-life process
- Facilitate difficult conversations and decision-making regarding end-of-life care
- Participate in interdisciplinary team meetings and contribute to the development of care plans
- Stay updated on current hospice care practices and attend relevant trainings and conferences
- Uphold Amedisys' values and standards of compassionate care in all interactions with patients, families, and team members
Master's Degree In Nursing From An Accredited Program: A Hospice Nurse Practitioner At Amedisys Must Have A Master's Degree In Nursing With A Specialization In Hospice Care. This Ensures That They Have A Strong Foundation In Advanced Nursing Practices And Knowledge Of End-Of-Life Care.
Active Nurse Practitioner License In The State Of Practice: Amedisys Requires All Hospice Nurse Practitioners To Hold An Active Nurse Practitioner License In The State Where They Will Be Practicing. This Ensures That They Are Qualified To Provide Patient Care Within Their Scope Of Practice.
Certification In Hospice And Palliative Care: Amedisys Seeks Hospice Nurse Practitioners Who Are Certified In Hospice And Palliative Care. This Certification Demonstrates Their Advanced Knowledge And Skills In Providing Quality End-Of-Life Care To Patients And Families.
Minimum Of 2 Years Of Experience In Hospice Or Palliative Care: Amedisys Prefers Candidates With At Least 2 Years Of Experience In Hospice Or Palliative Care.
